How and when you get a care assessment is very important to planning the services you receive at home. Getting it right, and developing the right care plan will make independent living much easier and more enjoyable.
The objective is to work out what help you need to live a safe and independent life. It should be on your terms, with your full consent and done so that you and the people around you fully understand every step of the care journey.
First, it is important to highlight that there is no charge for a care assessment. You are entitled to one no matter what income or savings you have, and no matter what your specific needs are. At Side by Side Care, there are no hasty decisions. We see our role as a partnership that enables you to live how you want. That’s very important to us.
Who does the care assessment?
The assessment will be carried out by a trained care specialist from Side by Side Care, face to face, in a relaxed environment where you feel comfortable. We also involve other healthcare professionals, from the local council, and maybe your doctor. This ensures that a more complete understanding of your care needs can be gained by everyone.
We make education and ongoing learning a priority. Our staff have extensive experience from a range of care settings, and this means you will always get a well-informed assessment.
Crucial to the process is that you will be involved every step of the way. We ask questions and listen. You and your family members will be given a chance to discuss any issues or difficulties you encounter when looking after yourself, and how this affects your daily life and overall wellbeing.
For example, you might find it difficult to wash and get dressed each day. Other people might struggle using appliances, such as the cooker or washing machine. It might be that you need some company, some reassurance and a helping hand to keep on top of weekly tasks.
The assessment of care needs extends to family members who help you in your day-to-day living. It is important that they are considered, so that they can also get assistance and guidance. The long-term aim is to make sure that everyone involved in your life is aware and confident that you are receiving outstanding levels of care and support.
